Indications
Pizotifen is used to treat vascular headaches of the migraine type, such as classical migraine, common migraine, and cluster headache, as a preventative measure. It is ineffective in stopping migraine attacks after they have started.
Pharmacology
The role of Pizotifen is to inhibit the peripheral effects of serotonin and histamine, increase the permeability of the cranial blood vessel membrane and the exudation of plasma kinin, and at the same time change the pain threshold of migraine. By blocking 5HT receptors, pizotifen attenuates serotonin signaling by causing cranial vasoconstriction and serotonin-enhanced platelet function and aggregation. There is evidence that it also inhibits the peripheral effects of bradykinin. Pizotifen can inhibit the reuptake of serotonin by platelets, thereby affecting tension and reducing passive dilation of extracranial arteries. The effect of pyrazotifen in causing appetite stimulation may be due to the drug acting on the metabolic level rather than directly stimulating the appetite center.
Dosage & Administration
Adults: Usually 1.5 mg daily. This may be taken as a single dose at night or in three divided doses. Dosage should be adjusted to the individual patient requirements up to a maximum of 4.5 mg daily. Up to 3 mg may be given as a single dose.
Children (aged over 2 years): Up to 1.5 mg daily, usually as a divided dose, although up to 1 mg has been given as a single dose at night.
Interaction
Pizotifen may augment the central effects of sedatives, hypnotics, antihistamines (including several common cold remedies), and alcohol. The hypotensive impact of adrenergic neuron blockers is counteracted by pizotifen.
Contraindications
Pizotifen is contraindicated in patients with known hypersensitivity to any of its ingredients.
Side Effects
The most common side effects are appetite stimulating effect, increase in body weight and drowsiness.
Pregnancy & Lactation
The use of Pizotifen during pregnancy is unknown, however no adverse consequences have been observed. As a result, the medicine should only be used during pregnancy if the potential benefit to the mother outweighs the danger to the fetus. Pizotifen has not been proven to be safe during nursing, thus mothers should avoid taking it during breastfeeding.
Precautions & Warnings
Renal impairment and urinary retention should be considered. Drowsiness can impair your ability to drive a car or operate machinery. Pizotifen boosts the effects of alcohol.
